In the military it was mandatory to get a flu...something. There was the nasal spray and the shot. Apparently, there was a method of blowing out the nasal spray after it was administered and avoiding your body "taking" the vaccine, but if this didn't work you where FOR SURE getting the flu. I always took the shot and I was usually fine after a day or so.
The injectable flu vaccine (typically trivalent) is a killed virus; the introduction of the virus into your system causes you to mount a response to the various strains in the vaccine, which sets you up to be resistant to further exposures to the particular flu strains. The intranasal vaccine is LIVE, attenuated (weakened) virus; again, it operates by causing you to mount a response.
The difference is that the shot generally just causes a mild inflammatory reaction (maybe some aches, pains and fatigue), while the nasal product actually "infects" you and inspires your system to come in for the kill and leave troops on watch. This infection is relatively mild, but it can have all the typical flu symptoms (at a much lower acuity) of getting the full-blown bug. The difference being that this attenuated bug is just a mild irritant while the full-blown flu can and does kill by overwhelming your respiratory system.
Now, as to the efficacy of vaccination...
The issue with vaccinating against viruses is that many of the damned things--our typical flu strains included--can adapt through mutation. When they mutate into a new virus, the likelihood is that the shot provides no protection from the newly-developed strain. Similarly, a mild strain from the wild that we didn't protect for can suddenly go virulent and get around the vaccination's protection. Thus the designing of
the flu shot each year is a reasearch-intensive exercise in probabilities--it comes down to the CDC folks having to make their best educated guess about what we will see in the real world in a given season.
If you've ever wondered why there doesn't exist a "cold vaccine", the above is your answer. There at any time are in the wild a multitude of different rhinoviruses, and they readily mutate for the most part. Thus, a cold vaccine would be really expensive to develop (as it would require growing all the major strains of rhinoviruses of infectious potential), and then would likely not work well because numerous viruses would mutate and bypass the vaccination's protection.
